Obesity is associated with several preventable health issues, such as diabetes mellitus and hypertension. Bariatric surgery has shown potential in treating obesity. Laparoscopic sleeve gastrectomy (LSG) one of bariatric surgical techniques gaining popularity a primary procedure. Situs inversus totalis (SIT) an uncommon hereditary abnormality that can present challenges laparoscopic due to the mirror-image anatomy. We case 54-year-old female body mass index (BMI) 54.36 kg/m2. She was diagnosed SIT had no other known diseases, medication use, or allergies. performed conventional LSG, modifying original trocar port positions match LSG safe effective procedure for patients SIT. Preoperative diagnosis help reduce risk complications by facilitating proper planning.
